Madison Lakes Dental is excited and proud to offer our services through our exclusive Dental Care Program. This membership allows easy and open access to dental treatment in a prepaid and discounted fashion.
This program is not dental insurance. Program guidelines are as follows:
• Program membership is only offered to individuals and families who are not currently insured by a dental health plan. All treatment for members must be completed at Madison Lakes Dental Dental. These benefits are not transferable to any other dental practice, specialty dental program, or office.
• Membership fees are one-time yearly payments that can be made with cash, check or credit card. Annual fees are due in full when signing up for membership.
• The one-year membership begins immediately upon payment.
• Members can decide which plan type is best for themselves and/or family members.
• Membership fees are non-refundable for any reason including, but not limited to, moving from the area or failure to utilize program benefits.
• Any payments for additional services beyond the preventive and diagnostic treatments included in the program are due on the day treatments are performed.
• We ask that you give our office a minimum of 24 hours’ notice if you must change a scheduled appointment.
• It is the responsibility of the Program Member to schedule their treatment appointments.
• Members are not eligible for any additional discounts, including senior discounts and paying the day of service discounts.
